Appointments
To schedule an appointment:
Please call us at 318-629-5555 Monday through Friday between the hours of 8:00 a.m. and 5:00 p.m.
For us to best treat your spine condition or injury, we need to have a comprehensive understanding of your current health status, Please bring all of the items listed below when you come in for your initial visit or consultation. If we do not have all of your medical records and the films that are related to your current condition at the time of your arrival, your appointment may need to be rescheduled.
Please bring the following items to your first appointment:
- All related medical records and diagnostic films (X-rays, MRI, CT Scans etc.) that pertain to your visit. These films can be scanned to a disk.
- A list of your current medications
- Current insurance card and driver’s license.
- A referral from your Primary Care Provider, if required
- Any brace or support that you are using

The required new patient paperwork can be downloaded and printed from the following pdf files. Please fax your completed forms to us at 318-629-5556 or bring them with you on the day of your appointment:
Medical Records
Please contact our Medical Records Custodian at 318.629.5557 or [email protected] for copies of your records or for a request to send records to another provider or facility
Financial Counseling
For any questions about your bill or to make arrangements for payment of the patient portion of your clinic or surgery expense, please call 318.629.4607 and we will be glad to assist you.
Workers Compensation
Our worker’s compensation liaisons work closely with both the patients and Workers Comp adjusters to ensure that a timely turnaround occurs from the physician’s order to approval of scheduling the procedure or surgery. Our liaisons can be reached at 318.629.5570
