Appointments

To schedule an appointment:

Please call us at 318-629-5555 Monday through Friday between the hours of 8:00 a.m. and 5:00 p.m.

For us to best treat your spine condition or injury, we need to have a comprehensive understanding of your current health status, Please bring all of the items listed below when you come in for your initial visit or consultation. If we do not have all of your medical records and the films that are related to your current condition at the time of your arrival, your appointment may need to be rescheduled.

Please bring the following items to your first appointment:

• All related medical records and diagnostic films (X-rays, MRI, CT Scans etc.) that pertain to your visit. These films can be scanned to a disk.
• A list of your current medications
• Current insurance card and driver’s license.
• A referral from your Primary Care Provider, if required
• Any brace or support that you are using
